Here where I live, every year we have an event called a Duck Derby, where they sell rubber ducks with sun glasses that you can send a float down the river on a particular day and with one of several fancy prizes.

I don't think that we will officially get an answer to this. Hell, I don't think that Joff knows, or wants to know for that matter. According to this story, Steve could be much more creative with his kills than what we are shown, especially since some of the things that Steve should, and would do, would scare me if Joff actually thought to put them on paper. Hell, if Joff thinks of some of the things Steve could do in his role of Death, pushed to the ends of his ingenuity, and came up with where I think that Steve should be, I WOULD BE SCARED OF JOFF!

A possible reason which could be used is someone dieing due to getting hit by a car or truck because they were picking up the "cute little rubber ducky". I can think of others which could happen, but aren't likely without Steve's direct involvement.
