No, it doesn't have to be a two-page spread. I think the more-or-less accepted definition of a splash page these days is any page which only has the one full-page image on it.
Purists, however, will tell you that even they are not [i]really[/i] splash pages. They are "single-panel pages", while the only [i]true[/i] splash pages are those which appear at the front of a book with the titles, credits, and publisher info on them. It all depends on who you're talking to and how picky you want to be about your definitions!
Oh, and the alternative to "[i]What the -[/i]" that I now tend to use wherever it seems appropriate is to drop the words all together and just go with a "- !? -" It seems to work in most scenarios!
