From what google says, it's an ivy cap or a newsboy cap.

The badge looks like it be easy to make with paper or felt cloth that you could pin/glue on.

The shirt:
Wear a black vest with a white t shirt under it. Problem solved. (If you can't find a suitable vest… get a black shirt and cut the sleeves off o_o)
